How GLP-1 medicines work in the body
Three mechanisms, explained plainly — so you know what the medicine does before you decide.
They act on hormone signals your body already uses
After you eat, your gut releases hormones that send signals to the brain, stomach, and pancreas. Semaglutide acts on the GLP-1 pathway. Tirzepatide acts on both GLP-1 and GIP pathways. Both medicines remain active for about a week, which is why they are taken once weekly.

They slow how quickly food leaves the stomach
These medicines can slow the movement of food out of the stomach, especially when treatment begins. This can help fullness last longer after a meal. The effect is usually strongest during the first few weeks and may lessen as the body adjusts.
They reduce hunger and appetite
They also act on parts of the brain involved in appetite. In controlled studies, people taking semaglutide or tirzepatide reported less hunger and ate less at a test meal than people taking placebo. Some people describe this as less “food noise” — fewer persistent thoughts about eating. The effect varies from person to person.

15–21% average weight loss
SURMOUNT-1 included 2,539 adults with obesity or overweight. After 72 weeks, average weight loss with tirzepatide ranged from 15% to 21%, depending on dose, compared with 3% with placebo.
What to keep in mind: Results vary, and weight regain is common after treatment stops.
SURMOUNT-1, New England Journal of Medicine, 202220% lower cardiovascular risk
SELECT followed 17,604 adults with overweight or obesity who already had cardiovascular disease. Heart attack, stroke, or cardiovascular death occurred in 6.5% of the semaglutide group and 8.0% of the placebo group — a 20% relative reduction.
What to keep in mind: This study involved people who already had cardiovascular disease. It does not establish the same benefit for everyone taking semaglutide.
SELECT, New England Journal of Medicine, 2023Lower progression to type 2 diabetes
A three-year extension of SURMOUNT-1 followed adults with obesity and prediabetes. Type 2 diabetes developed in 1.3% of those taking tirzepatide and 13.3% of those taking placebo while treatment continued.
What to keep in mind: This finding applies to people who already had prediabetes. The difference became smaller after treatment stopped.
SURMOUNT-1 three-year results, New England Journal of Medicine, 2024Less hunger and fewer cravings
A 12-week study followed 30 adults with obesity. People taking semaglutide ate 24% less at a test meal and reported less hunger and fewer cravings than people taking placebo.
What to keep in mind: This was a small, short study. Appetite changes vary from person to person.

STEP 1 included 1,961 adults with overweight or obesity. After 68 weeks, people taking semaglutide 2.4 mg reported greater improvement on a physical-functioning questionnaire than people taking placebo.
What to keep in mind: This was a patient-reported measure, not a direct test of strength, fitness, or energy.
STEP 1, New England Journal of Medicine, 2021Fewer sleep-apnea events
SURMOUNT-OSA studied adults with obesity and moderate-to-severe obstructive sleep apnea for 52 weeks. Tirzepatide reduced breathing interruptions by about 25 to 29 events per hour, compared with about 5 per hour with placebo.
What to keep in mind: This result applies to people diagnosed with moderate-to-severe obstructive sleep apnea.
